John is a 14-year-old boy with perinatally acquired HIV infection. He was diagnosed at 5 years of age, and has received antiretroviral therapy since that time. Over the past several months he has had progressively worsening pain in both lower extremities. He describes the pain as a shooting or burning sensation.
